Senior Director of Supportive Housing

Position Summary:

The Senior Director of Supportive Housing will oversee all case management for the Permanent Supportive Housing (PSH) and Rapid Rehousing (RRH, YHDP) programs as well as Homes with Hope's after school program (ASAP). As part of Senior management, this position will assist in the development and implementation of new and existing HwH programming, supportive housing, and enrichment services.

Case Manager - Permanent Supportive Housing

Position Summary:

This Case Management position reports to the Director of Supportive Housing and will provide case management to formerly homeless individuals with a diagnosed mental illness. The Case Manager will provide case management assistance to the supportive housing tenants using an "assess and refer" model which will include outreach, assessment, referral, crisis intervention and ongoing life-skills training. The Case Manager will also work collaboratively with the building's Property Management to foster successful tenancy.

Director of ASAP (After-School Academic Program)

Position Summary:

The Director of Homes with Hope's after-school program (ASAP) is responsible for the operation of a small site-based after-school program for children and teens who live in Homes with Hope's supportive housing and the surrounding neighborhood. The Program Director's (PD) goal is to provide a consistent structure, positive role models, academic support, and enrichment activities to help ASAP students develop the skills they need to be successful at school and in their personal lives. This is a part-time position, for around 25 hours per week.

Program Director - Mentor Initiative

Position Summary:

Mentoring Initiative is a community-based program whose mission is to provide individuals with one-to-one and potentially group support to help them develop a stronger sense of self-worth and encourage them to create a self-sufficient, successful life for themselves and their families. This is done through carefully developed mentoring partnerships between trained volunteer mentors and individuals who are in need and are struggling to get by. Through these partnerships, mentors have an opportunity to make a significant difference in another person's life.
